OPT OUT Mail Campaign
This is an OPT OUT page for our 2024 mail campaign.
We want you to know of this mail campaign, so you can decide if you want to OPT OUT or not.
This campaign will allow Pacific Group Agencies to mail you a booklet and information on the insurance packages they offer. These insurance packages include innovative retiree group and individual programs for health, dental, vision, hearing, pre-paid legal and life coverage.
IF you wish to OPT OUT, make your selection in the box below titled "MCARE - OPT OUT FORM 24-1". NOTE: IF you have previously sent this form in prior years, you do NOT have to submit it again.
